All full-time employees receive the option of joining our long term disability plan.
Employee Voluntary Long Term Disability Insurance
The effective date is when your initial enrollment begins, which is the first day of the month after completing a 90-day waiting period. If you choose to waive this benefit when first eligible, you will be able to enroll in this benefit during open enrollment but you will be subject to approval by the insurance company after completing the evidence of insurability form.

Amount of Insurance: 60% of your total monthly earnings, not to exceed the maximum monthly benefit of $5,000.00

The minimum monthly benefit is $100 or 10% of the gross monthly benefit, whichever is greater.

Elimination Period: 90 days - a period of time you need to be continuously totally or partially disabled.

You will receive notifications via our Employee Portal (UKG) as a checklist within the first 14 days of employment. Please ensure that you complete the required benefit checklist even if you choose to waive insurance.
