All full-time employees receive the option of joining one of our fully comprehensive Health insurance plans.

 

We offer two (2) PPO (Traditional Co-pay) plans as well as three (3) High Deductible Health plans (Also known as HSA plans) within two (2) different Networks.

Who is Covered

Each full-time employee receives a comprehensive health insurance package for themselves, their spouse, and dependents.

Effective Date for New Hires and Employees Promoted from Part time to Full Time

The effective date is when your initial enrollment begins, which is the first day of the month after completing a 30-day waiting period as a full-time employee.

Benefit summaries should be reviewed carefully. The same benefit summary is also available during your enrollment process on our Employee Portal (UKG). 

You will receive notifications via our Employee Portal (UKG) as a checklist within the first 14 days of employment. Please ensure that you complete the required benefit checklist even if you choose to waive insurance.

Health Savings Accounts

A health savings account (HSA) can help you lower your taxes, pay for health care more easily and even save for retirement.

HSAs are only available with high-deductible health plans. You can use HSA funds to pay for eligible health care expenses and for out-of-pocket costs your health plan doesn’t cover.

PLEASE NOTE

1) Hirshfield's does NOT provide FSA debit cards

2) You can only use the HSA with the HDHP (High Deductible) plans

3) You can only use the Full FSA with the PPO  plans - Receipts for reimbursement have to be submitted to HR monthly and will be reimbursed via Direct Deposit as indicated on your payroll Direct Deposit, unless requested differently in writing to HR.

4) You can establish a LIMITED FSA with the High Deductible plans for Dental and Vision only--Receipts for reimbursement have to be submitted to HR monthly
